I usually tend to use LOL, lol, lmfao, or any variation to add a good spirit into whatever I'm typing. Like in order to keep the conversation in a happy tone I add lol. If you are talking to a friend and say something like, "Ya, I can't believe I missed that shot," your friend may read that and think you're really bummed. However if you don't really care and feel light hearted about the subject you might say, "Ya, I can't believe I missed that shot lol." he'll know that it wasn't a big deal. however, usually if I say LOL or LMFAO and it's capitalized I really did smirk, chuckle or laugh uncontrollably. lol

Basically lol. I'm not sure I know of anyone who uses it like that, except for Mik I guess. As stated by his post on here lol

I laugh out loud when I type 'lol' most of the time, and when I don't I add 'lol' to show I am not being serious, or I am joking or to lighten up a mood when things get kinda tense. But mostly I do laugh. But you know me Zilf, I laugh alot. *feels like laughing for just thinking about laughing* I guess I agree with Lord Abel.
